This is an exciting opportunity to work with a well-funded healthcare startup on its patient care coordination team. Patient Care Coordinators are responsible for liaising between internal and external providers, patients, and clinical staff. The Patient Care Coordinator will be responsible for establishing and maintaining the relationship between Assure Health and the qualifying patients and their caregivers. The ideal candidate will be deeply empathetic and able to answer potential patients’ questions thoroughly and thoughtfully. We prefer this role to be based in South Florida, but are open to it being remote for the right candidate.
Triage phone messages from patients and other medical practicesWill be one of the key touch points to patients representing Assure Health and will answer questions for patients, help them understand the process, and walk them through various processesProcess patient information into EHR and other software used by AssureAssist in the coordination of care with patients PCPs and specialists, which will include patient data sharing and scheduling between Assure clinicians and other PCPs and specialistsConduct follow-up and reminder phone calls to patientsHelp patients troubleshoot technology issues with device or audio/video appointmentsManage practitioners’ schedules and assist with coordination of patient schedulingBecome proficient in use of DrChrono and other care software leveraged by Assure Health to assist in triage and documentationManage the device ordering and replacement processAssist in the insurance verification process, which will include checking eligibility with insurers and in some cases trying to secure prior authorization
Assist as requested by providersProvide general administrative support to providers and management
Skills and Requirements:
2 years of previous experience as a patient service representative (or similar role)Able to be empathetic towards patients with chronic conditionsWorking knowledge of Salesforce CRMExcellent communication and presentation skillsProficient in English and SpanishAble to negotiate and deal with complaintsPersistent in a kind manner and results-orientedAble to work independently
